I hereby consent for Surfside Yoga Studio to take photographs of my child and grant permission for the reproduction of the photographs in promoting My Yoga Playground on social media (Facebook and Instagram), its website and print material without payment. I understand that it is my responsibility to update Surfside Yoga Studio staff in the event that I no longer wish to authorise the above uses.

By agreeing below, you are consenting to your child participating in yoga led by Catherine Tong from My Yoga Playground, or a qualified substitute teacher and acknowledge that you have disclosed any past or present injury, illness, health problem or condition that your child has or have previously had that may affect their participation. Yoga is not a substitute for medical attention, examination, diagnosis or treatment. By agreeing below you are responsible in deciding that your child practices yoga, and hereby agree to irrevocably release and waive any claims that you have now or hereafter may have against Catherine Tong of MYP or any member of the Surfside Yoga team.
